Personal Planning (a.k.a. Roles Based Planning) is a personal approach with the purpose of achieving positive outcomes for adults by increasing their skills, opportunities, roles, image, and relationships within the community.
Roles Based Planning draws upon 25-years of learning accumulated though the following schools of thought: Individual Rights; Self-Determination; Person Centered Planning; Social Role Valorization; Relationship Facilitation; and Positive Behavior Support. Roles Based Planning, however, moves beyond the traditional scope of each school of thought by using all of them in a carefully balanced combination that provides a holistic approach to personal planning.
The Personal Planning process includes the completion of a referral form by DDRC's community support team and an annual meeting between an individual and his or her support network. The outcome of an initial meeting includes the establishing of concrete personal goals, indicators of success, and initial action steps for the coming year. Subsequent action steps will then be developed and carried out by the community support team. Planning coordinators will also be available on an as needed basis throughout the year.
Cost
DDRC provides this service at no charge to current clients of DDRC to complement the daily support given to each individual.
